This form acknowledges your possession of LCISD equipment and assumption of financial responsibility for the equipment usage off school property or outside of a school-sponsored event.
You agree that you will return all equipment in your possession to the District [upon termination of my employment] OR [at the end of the current school year] in good condition.
You agree that you will be responsible for any loss of or damage to the equipment. You also acknowledge LCISD has not required that you enter into this agreement as a condition of your employment with LCISD.
You understand that this equipment is subject to the rules, regulations and policies governing LCISD Schools, including Board Policy CQ(LOCAL). You understand that all facets of the Acceptable Use Policy are in full effect.
